Listed below are some of the forms that are commonly requested by veterans.  These forms are being made available in case coming to our office during working hours is difficult for you. However, we are here to help you with them if you prefer that.  

 VA Forms

  • Health Care Enrollment Application (VA 10-10EZ) - This is the application for enrolling in VA Healthcare. All information is based off the previous calendar year's income and it must be exact. If it gets approved based on incorrect information, the VA may come back years down the road and charge you services you received but weren't eligible for.
  • Change of Address (20-572)
  • Intent to File (21-0966)
  • Release of VA Health Records to Private Medical Facilities (10-0485) - This is required for sharing information when using Community Care
  • Statement in Support of Claim (21-4138)

Home Buying

VA home loan guarantees are issued to help eligible service members, veterans, reservists and unmarried surviving spouses obtain homes, and to refinance loans.  To learn more about VA guaranteed home loans, click here.

Miscellaneous Services

  • Obtain Military Awards, Military Records Request and Medals Replacement (SF180)
  • Obtain a National Disabled Veteran Parks and Trails Pass (NPS Form 10-596)

Veterans Service Commission/Emergency Assistance

The purpose of the Veterans Service Commission is to provide a one-time emergency assistance or re-payment loan under exceptional circumstances to Veterans who reside in Marathon County and to family members of deceased Veterans who reside in Marathon County.

Applicants must be a permanent resident of Marathon County for a minimum of 12 months. Veterans must meet legal definition of a Veteran as defined in WI Statutes 45.01(12).

To apply for assistance, applicants must complete and submit the online application. The application submission will be reviewed by the Veterans Service Officer and considered by the Service Commission with an approval/denial decision provided to the applicant within 10 days.

If approved, assistance is provided directly to a landlord, utility company, vendor, etc. Additional resources are also provided unique to the applicant’s circumstance with the goal of financial stability and self-sufficiency.
